Re-Irradiation in Recurrent High-Grade Glioma

چکیده
Background and Purpose: Despite the progress in treatment modalities such as neurosurgery and radiotherapy, almost all patients treated with malignant gliomas develop recurrent tumors and die of their disease. The aim of this study was to determine response, survival and treatment toxicities after reirradiation of relapsing primary malignant brain gliomas. Material and Methods: Twenty patients with recurrent or progressive primary brain glioma were subjected to re irradiation. 3-D conformal radiotherapy with a median target total dose of 35 Gy (BED = 66.5 GY 2) was applied using 1.8-2 Gy daily; 5 days per week with a 6 MV linear accelerator. Results: Patients were assessed as regard response, survival and treatment complications. Median progression free and overall survival was 3 and 9 months respectively. As regard subjective response; 50% of cases showed improvement symptoms. Radiographic indices were stabilized or improved in about 60% of patients. Brain necrosis was noted in 10% of patients. Conclusion: Re-irradiation of recurrent brain gliomas allows the survival prolongation and delays disease progression or recurrence, with acceptable toxicities.

Re-irradiation alternatives for recurrent high-grade glioma
Despite advances in the fields of surgery, chemotherapy and radiotherapy, the prognosis for high-grade glioma (HGG) remains unsatisfactory. The majority of HGG patients experience disease recurrence. To date, no standard treatments have been established for recurrent HGG. Repeat surgery and chemotherapy demonstrate moderate efficacy. As recurrent lesions are usually located within the previousl...
متن کاملA Review of the Role of Re-Irradiation in Recurrent High-Grade Glioma (HGG)
Despite the use of more effective multimodal treatments in high-grade glioma (HGG), the outcome of patients affected by this disease is still dismal and recurrence is a very common event. Many therapeutic approaches, alone or combined (surgery, drugs, targeted agents, immunotherapy, radiotherapy, supportive therapy), are available in the clinical armamentarium so far. The attitude of physicians...
متن کاملRe-irradiation for recurrent glioma- the NCI experience in tumor control, OAR toxicity and proposal of a novel prognostic scoring system
PURPOSE/OBJECTIVES Despite mounting evidence for the use of re-irradiation (re-RT) in recurrent high grade glioma, optimal patient selection criteria for re-RT remain unknown. We present a novel scoring system based on radiobiology principles including target independent factors, the likelihood of target control, and the anticipated organ at risk (OAR) toxicity to allow for proper patient selec...
متن کاملExternal beam re-irradiation, combination chemoradiotherapy, and particle therapy for the treatment of recurrent glioblastoma.
Glioblastoma is a common aggressive primary malignant brain tumor, and is nearly universal in progression and mortality after initial treatment. Re-irradiation presents a promising treatment option for progressive disease, both palliating symptoms and potentially extending survival. Highly conformal radiation techniques such as stereotactic radiosurgery and hypofractionated radiosurgery are eff...
متن کاملManagement and treatment recommendations for World Health Organization Grade III and IV gliomas
The treatment recommendations provided in this manuscript are intended to serve as a knowledge base for clinicians and health personals involved in treating patients with high-grade malignant glioma. In newly diagnosed patients, complete resection or biopsy is required for histological characterization of the tumor, which in turn is essential to decide the treatment strategy. In patients with g...
